EH - Annual Review for EHC Plans

This 2-hour session gives an in-depth explanation and an opportunity for a shared discussion about co-production meetings after an Education, Health and Care Needs Assessment has been completed and ways to co-produce the Education, Health and Care Plan (EHCP).

This session is aimed at: All practitioners (education, health and social care practitioners) working with children and young people with an EHCP aged 0-25 years.

What will be covered:

• An in-depth explanation of the legal timeframes and obligations in the process of reviewing EHC Plans.
• Providing advice for Annual Reviews.
• A look into how phase transfers work.
• Circumstances in which schools and/or parents should call an early annual review.
• Preparation for Adulthood.
